How to Join Our Chapter

1. ​Visit the PGSA website at WWW.PGSA.ORG.​
2. Click the MEMBERSHIP box (the first box on the far left of the Home Page)
3. The PGSA MEMBERSHIP BENEFITS & APPLICATION page will appear.
4. Scroll down to the caption “PGSA MEMBERSHIP OPTIONS”. This chart lists the types of membership that are available with their respective dues.
5. Directly under the “PGSA MEMBERSHIP OPTIONS” box are two methods of membership registration and payment for New Members.
6. You will use the appropriate instructions.
7. Choose the membership registration process you prefer:

Join Online: Clicking the Join Online enables you to enter all of your data and pay online using your Credit Card or Pay Pal. IMPORTANT: In the Comments Box, be sure to type PGSMA or Massachusetts.  

Join Using the Paper Form:​ 

1. "Click Here to Print the New Membership Form (PDF)" provides you with printing a paper registration form and making payment by check. 
2. Click the title to show the form for printing, then print the form.
3. Complete the form, being sure to select your desired membership type And in an open space be sure write PGSMA or Massachusetts.
4. Include your check with the applicable amount made payable to PGSA 
5. Mail the application and check to: PGSA – c/o Phyllis Klen 2816 Hillcrest Circle Naperville, IL 60564  ​
